Abstract
The CDC’s Division of Diabetes Translation Diabetes Primary Prevention Initiative is a collaboration between the division and five state Diabetes Prevention and Control Programs. The goal of the prevention initiative is to develop a conceptual framework that will lead to:
- New ways that Diabetes Prevention and Control Programs can implement interventions to prevent type 2 diabetes;
- Nontraditional strategies and partnerships for type 2 diabetes primary prevention; and
- Acceleration and adoption of practices, new tools, models, and methods to serve as a process roadmap for
Diabetes Prevention and Control Programs.
This article describes how the division facilitates nationwide surveillance and reduction of the diabetes burden through the work of the state programs, the partners and unique initiatives utilized in the five states to translate the research of diabetes primary prevention into practice. Challenges and lessons learned are also explored.
