Enhanced by Technology: A New Level for Chronic Disease Self-Management Current intervention for patients with chronic disease has been face-to-face. Given the number of people with diabetes, other methods are needed to reach and intervene.The session will provide evidence-based concepts of successful technology-enabled management, knowledge needed to understand how to implement a cellular- and internet-based program that leads to cost effective and improved outcomes.Policy context to increase reimbursement for technology enabled support for patients with chronic diseases will also be discussed.

Objectives:
- Identify the key elements of behavior change interventions and how these elements can be delivered using internet and cell phone technology.
- List the evidence-based principles that lead to successful technology-enabled diabetes self-management support interventions.
- Discuss the public policy change needed to increase reimbursement for technology enabled self-management support interventions for patients with diabetes and other chronic diseases.
